The Informed Consent & Client Agreement is built into the booking process — no need to print or submit it separately. If additional guests are joining your session, please share this page with them in advance. By participating, all guests acknowledge they have reviewed and agreed to its terms.

The Media Release & Authorization Form is separate and entirely optional. It will only be requested if you choose to share a testimonial or permit use of a recorded clip. If your session included additional guests, please note that each person present in the recording must submit their own Media Release & Authorization Form. A release from one participant does not cover others who appear in the same recording. The Media Release & Authorization Form can be completed at the bottom of the Testimonials page.
